They are registered, insured, by the states. You can't just buy a car at a carshow and then drive it. You must re register it.

only certain people can drive a car based on their skills and physical ability and age.

only certain people can drive certain types of vehicles based on training.

commercially, vehicles and driving time are heavily regulated.
