just think about it:
it makes sense, they even admit there was a drill, they thought it still was a drill when bombs went off.
i don´t buy the "lone wolf w/ pressure cooker" story, i suppose this was made up to hide that actually that drill went really really wrong.
this would explain as well why not that many people got injured/are dead, and why it took place after the best/prominent runners have finished long ago - the explosives did not do much damage at all (less than another type of bomb and another, much earlier time would have done)
as there was less people/watchers for the last guys running, so less injuries.
