Original Message A newly identified personality disorder has been added to the DSM IV. While this disorder has been exhibited for decades, if not centuries, and includes many characteristics of personality disorders already included in the DSM, mental health professionals agreed that the surge in prevalence and increased severity of pathology of this disorder, as seen in recent years, warrants this disorder being given it’s own name and entry in the DSM at this time.
Taking into consideration the wide range of abhorrent and despicable behaviors displayed by those afflicted with this disorder, it was unanimously agreed that the most appropriate name for this new disorder is, Pathological Scumbag, which shall be referenced as PS for the remainder of this article.
The PS has combined characteristics of the Pathological Liar and Sociopath, with an overriding presentation of Narcissistic Personality. There are, however, some excluding behaviors which prevent the assignment of these diagnoses and create the need for the new diagnosis of Pathological Scumbag.
First, you will see the Narcissistic Personality. Narcissistic personality is characterized by behavior or a fantasy of grandiosity, a lack of empathy and a need to be admired by others. Narcissistic personality has a pathological unrealistic or inflated sense of self-importance, has an inability to see the viewpoints of others, and is hypersensitive to the opinions of others. Meeting five of the following criteria indicates the presence of this disorder.
1. Grandiose sense of self-importance.
2. Fantasies of and preoccupied with beauty, brilliance, ideal love, power, or unlimited success.
3. A belief of being special and unique and can only be understood or a need to associate with people of high status.
4. A need for excessive admiration.
5. An unreasonable expectation of being treated with favor or excepting an automatic compliance to her / his wishes.
6. Will use others to achieve her / his goals.
7. Lacks empathy.
8. Believes others are envious of her / him or is envious of others.
9. Contemptuous or haughty attitudes / behaviors.

In the PS, you will typically see all 9 criteria met with emphasis on 1, 4, 5, 6, and 8.

Layered on top of the Narcissistic Personality, the PS will meet many criteria of the Pathological Liar. The difference is, where the Pathological Liar lies for no reason – not to gain advantage, impress, or hurt others, (See [link to thelastpsychiatrist.com] ).
the PS lies for all of these reasons and seems to often be connected to criteria 6 – will use others to achieve her/his goals. The PS lies intentionally, with premeditation, and purposefully, which prevents the diagnosis of a Pathological Liar, who can’t seem to help themselves.

Another layer of pathology includes some of the characteristics of the sociopath. A good description of this disorder can be seen here: [link to www.hss.caltech.edu] I will only reference the pertinent characteristics as seen in the PS. These include but are not limited to:
• Glibness and Superficial Charm
• Manipulative and Conning
They never recognize the rights of others and see their self-serving behaviors as permissible. They appear to be charming, yet are covertly hostile and domineering, seeing their victim as merely an instrument to be used. They may dominate and humiliate their victims.
• Grandiose Sense of Self
Feels entitled to certain things as "their right."
• Shallow Emotions
When they show what seems to be warmth, joy, love and compassion it is more feigned than experienced and serves an ulterior motive. Outraged by insignificant matters, yet remaining unmoved and cold by what would upset a normal person. Since they are not genuine, neither are their promises.
• Does not perceive that anything is wrong with them.
• Only rarely in difficulty with the law, but seeks out situations where their tyrannical behavior will be tolerated, condoned, or admired
• People with this disorder appear to be charming at times, and make relationships, but to them, these are relationships in name only. They are ended whenever necessary or when it suits them, and the relationships are without depth or meaning, including marriages. They seem to have an innate ability to find the weakness in people, and are ready to use these weaknesses to their own ends through deceit, manipulation, or intimidation, and gain pleasure from doing so.

So one may ask, why not assign the diagnosis of Sociopath. The excluding behavior is that the PS feels remorse and guilt, which turns to shame, causing the PS to turn on the people who have been lied to and deceived in an effort to cover up the lies and sociopathic behavior creating an endless cycle of scumbag behavior that reaches the level of pathology in a very short period of time. A true Sociopath does not feel shame or remorse. And it is this difference that creates the Pathological Scumbag – the deep rooted shame and knowledge of their own despicable behavior and treatment of others causes the PS to continue to lie, manipulate, and deceive, in order to maintain the illusions of the Narcissistic Personality, which is propped up by the other identified personality disorders (with excluding behaviors noted), this resulting in a disorder in a class of its own, the Pathological Scumbag.

Studies have been conducted to find a treatment for this disorder but none have been successful. It seems the best thing to do is isolate the PS and remove the PS from all positions of power that support the Narcissistic delusions as a first step. If it were possible to put the PS on an island alone, where the PS would be forced to end the self-perpetuating cycles of pathology, there may be some improvement in the overall presenting behaviors. Since this is not possible, the prognosis is quite dismal for the Pathological Scumbag. Most people diagnosed with PS continue to lead a very sad and pathetic existence, eaten up by shame that is only alleviated by brief moments of exuberance when another scheme and manipulation has been successful in reaching a new goal. As that brief moment of pleasure fades into self-loathing, the PS must find a new scheme, a new victim, involving more lying and immoral behavior, to get the pleasure need filled. You can see how this is never-ending. It is generally agreed that the normal treatments of disorders are useless with the PS and professionals are encouraged to end all therapeutic interventions as soon as the diagnosis of a Pathological Scumbag has been identified.
