Digital Cable Adapter for TV and Cable...to get or not to get?

I can't seem to find the info on the conspiracy of those adapter boxes for the HD tvs. We have a flat screen tv, and now we are losing cable channels b/c we didn't get the adapter box.

Is this the same as the digital hd box that has recording devices?
