Is Ophiuchus the 13th zodiac sign?
No. Ophiuchus is just a constellation. Even the founding text for astrology (Tetrabiblos by Claudius Ptolemy, written 170 AD) mentions just 12 signs. Though a very small fraction of western astrologers who follow a different, constellation-based zodiac, or those who follow Vedic astrology, may use Ophiuchus as the 13th zodiac sign, or assign different dates to the signs used in western astrology.

It's true that the Tetrabiblos didn't mention Ophiuchus as a constellation, but the Almagest did. But that's all irrelevant anyway since Ptolemy didn't have anything to do with astrology, he only listed constellations.

The Babylonians started astrology, and yes they did originally include Ophiuchus as a zodiac sign.

Let me rephrase that: the Babylonians originally included Ophiuchus as an astrology sign.

The term zodiac doesn't necessarily imply astrology, it also has a meaning in mathematical astronomy.

(The fact that certain stars had become connected with the schematic year of 12 months x 30 days each greatly assisted the development of the reduction of the zodiac to 12 divisions. The calendar was schematic because of the fact that the year does not consist of exactly 360 days. This made it necessary to add an extra 13th month now and then.

The periodic intercalation of a 13th lunar month was done to keep the lunar calendar in line with the seasons.
