Northeast Tennessee is experiencing an E. coli outbreak, with 10 cases of the infection reported in the past week, two more than had been reported on Monday and nearly twice the number reported in the region during all of 2010.

“We are taking it very seriously and encouraging people to follow the precautions,” Dr. David Kirschke, medical director of the Northeast Tennessee Regional Health Office, said.

Based on lab results from eight cases reported to regional health office on June 2 and 3, Kirschke said, “It looks like is we may be dealing with two separate (strains). What it looks like is seven cases that are not related to the 0157 (strain) that are probably a milder strain, and one case of the 0157 that can have more severe complications.”

Kirschke said it takes several days for cultures of toxin samples taken from infected patients to grow, and more will be known as the lab results on those toxins comes in.

He said the 10 reported cases are spread out fairly evenly over four Northeast Tennessee counties and do not include two Southwest Virginia children who were brought to Johnson City Medical Center with E. coli infection over the weekend.

The Virginia Department of Health confirmed Monday that one of those children died from complications from an infection of 0157 E. coli, while the second child, who the VDH would only identify as “a person in close family contact with the child who died,” has been hospitalized. A JCMC spokesman said Monday the second child was transferred to another hospital.

While department of health officials and JCMC have refused to release any further information about the siblings, other sources have confirmed the child who died was a 2-year-old Dryden, Va., girl and that her brother was flown to Vanderbilt University Medical Center in Nashville for treatment of similar symptoms.

Kirschke said the 10 Northeast Tennessee cases also include a young child who was receiving care in the intensive care unit of a Knoxville hospital Monday.

While young children do appear to be more susceptible to complications, Kirschke said, typically only 5 to 10 percent of all patients infected with the 0157 E. coli strain experience any complications.

He said the regional health office is “interviewing and re-interviewing” all of the local patients’ family members in attempt to identify any “common exposures” that could be used identify a source of the bacteria, such as under-cooked hamburger, fresh fruits or vegetables that have not been thoroughly washed, recreational waterways or petting zoos.

According to Kirschke, the health department investigation so far has not revealed any common factors among the patients that could be linked to a possible source. He said the lab results received so far also do not indicate any similarity between local cases and the E. coli outbreak in Europe that has killed 24 people and in infected more than 2,400 others.

If a source can be identified, Kirschke said the health department will advise the public of specific precautions to take. In the meantime, the department is making general recommendations to help avoid exposure, including thoroughly cooking hamburger, thoroughly washing fresh fruits and vegetables and being careful not to swallow while swimming in natural waterways. Kirschke also recommended being especially careful with young children because of the difficultly of preventing them from swallowing while swimming.

Because E. coli infection and many other illnesses are most often associated with contaminated food or water or contact with other people or animals that have the bacteria, Rebecca Bartles, manager of infection prevention for Mountain States Health Alliance, stressed the need for good “hand hygiene.”

“It’s especially important in food preparation, making sure meat is handled properly and cooked correctly,” she said.

Bartles also advised caution while swimming, especially in water that has not been treated with antibacterial chemicals.

I second that MR, I just went docs yesterday to find out the results of my bloods and urine tests done last week. Shows in Urine that I have E Coli infection.

I have very slight pain in kidneys and feel bit sick, they gave me Macrodantin 50mg Capsules which contain Nitrofurantoin Antibiotics. Got to go back in 7 days for more tests.

If anyone does suspect it, get professional help straight away.

I'm in Manchester UK, in europe where the outbreak is here, 22 people died so far from it. Don't know if mine is related yet.

I went docs initially because ive been having this pain under my last rib on right. They wanted to check my liver as my liver enzymes were up last year, then they found the e coli lol, think i may of had a dodgy kebab couple of weeks ago.

Funny you say this, had a kebab with salad from an Indian takeaway few weeks ago, now I got E Coli lol, just saying..

ATLANTA – More Americans got food poisoning last year, with salmonella cases driving the increase, the government reported Tuesday. Illness rates for the most common serious type of E. coli fell last year. There was a rise in cases caused by other strains of the bacteria, although that bump may just reflect more testing was done for them, the Centers for Disease Control and Prevention said.

An unusually aggressive strain of E. coli is behind the current large outbreak of food poisoning in Europe, mostly in Germany. That strain has never caused an outbreak in the U.S.

The CDC estimates that 50 million Americans each year get sick from foodborne illnesses, including about 3,000 who die.

The report released Tuesday is based on foodborne infections in only 10 states, or about 15 percent of the American population. But it has information that other databases lack and is believed to be a good indicator of food poisoning trends.

More than 19,000 cases of food poisoning were reported in those states last year. That was up from 17,500 cases in 2009, and about 18,500 in 2008.

Last year, there were 4,200 hospitalizations and 68 deaths in those states.

Year-to-year numbers can be misleading, especially from just a sample of states. Health officials note that the number of food poisoning cases have decreased by about a quarter since tracking began 15 years ago. Rates for most of the illnesses have also been relatively flat.

Not for salmonella, however. The bug caused the most illnesses of the nine leading food-poisoning causes last year. Salmonella cases haven't diminished in 15 years, and rose in the last few years by 10 percent.

"We've made virtually no progress against salmonella," said CDC Director Dr. Thomas Frieden.
One of the largest U.S. outbreaks last year involved salmonella tainted eggs that may have sickened as many as 56,000, according to a CDC estimate. That probably contributed to the increase seen in Tuesday's report, said Dr. Christopher Braden, a CDC epidemiologist.

The Food and Drug Administration last summer put in place new rules that should significantly reduce illnesses caused by salmonella in eggs, the FDA's Michael R. Taylor said.

Officials hope to put the same kind of dent in salmonella that they did with E. coli O157. The bacteria became infamous in a 1993 outbreak linked to Jack in the Box hamburgers.

More regulation and testing of meat helped cut those E. coli cases in half — from a rate of 2 per 100,000 people to less than 1 per 100,000 last year.

In the bad news department, health officials continue to see jumps in illness caused by a group of bacteria called vibrio, which are associated with shellfish. There were fewer than 200 vibrio cases reported in 2010, but that's more than double the numbers seen in the 1990s.

Vibrio cases are preventable. Flash-freezing and pasteurization of oysters could reduce the risk to consumers, Braden said.
