New cases of H7N9 have not been detected in China since May this year, until now.

Here we have the detection of a new and serions case of H7N9 after all this time, which could be signalling a new outbreak.

It is interesting to note that ealsier commentary from the medical community did signal a level of expectation for this bug to re-emerge.

Lets hope that this is an isolated case, and not the beginning of a fresh outbreak which could still lead to a world wide pandemic as is also feared by those medical experts.

A 61-year-old female has been confirmed as a case of H7N9 avian influenza virus infection in Beijing by Beijing CDC on Sat 20 Jul 2013. She is a resident of Langfang Shi, a city between Beijing and Tianjing [in Hebei Province]. She developed fever on 10 Jul 2013 and was diagnosed as a case of severe pneumonia on 13 Jul 2013. She was transferred to Beijing Chaoyang Hospital on 18 Jul 2013. The patient is in serious condition and being treated.

An update on the MERS-coronavirus sitution follows here.

New cases continue to present and are also now being detected in asympomatic people.

While it is good to see this better surveilance and to understand that the CFR (case fatality rate) is lower than first thought to be, it still doesnt change the fact that this coronavirus causes long term serious and life threatening illness in a significant number of people.

Worse still are the facts that the reservour and vectors for this coronavirus continue to remain veiled from our understanding, all the while it continues to distribute itself over ever wider areas, now established in Abu Dhabi, and inspite of the WHO's rediculous attitude of not acknowledging and attempting to manage this menace, it continues to establish a very dangerous foothold from what is certain that it will rapidly become a world wide pandemic with the onset of the Haag now very fast approaching and with pilgrims certain to have locked in their travel plans to attend this very significant religous festival, by now.

This is a tragedy of biblical proportions in the making.

I wonder how the WHO will respond once the inevidible pandemic actually emerges and is responsible for unimaginable pain, suffering and death that could have been avoided.

Will the world at learge continue to tollerate the continuance of the WHO after an event like the approaching disaster and for which they have direct responsibility for the inevidible outcome of possibly tens of thousand of deaths.

I certainly hope that the individuals who represent the WHO and are making these decisions at this time will be personally held to account for their culpable actions.


Middle East respiratory syndrome coronavirus (MERS-CoV) - update 18 Jul 2013

WHO has been informed of 6 additional laboratory-confirmed cases of infection with Middle East respiratory syndrome coronavirus (MERS-CoV). Of these, 2 cases have been reported from Saudi Arabia and 4 from the United Arab Emirates (UAE).

Both the cases in Saudi Arabia have mild symptoms and are not hospitalized. They are from Asir region. The 1st case is a 26-year-old man who is a close contact with a previously laboratory-confirmed case, and the 2nd case is a 42-year-old woman who is a health care worker.

In the UAE, the 4 cases are health care workers from 2 hospitals in Abu Dhabi who took care of an earlier laboratory-confirmed patient. Of these, 2 cases, a 28-year-old man and 30-year-old woman, did not develop symptoms of illness. The other 2 cases, both women of 30 and 40 years old, had mild upper respiratory symptoms and are in stable condition.

Globally, from September 2012 to date [18 Jul 2013], WHO has been informed of a total of 88 laboratory-confirmed cases of infection with MERS-CoV, including 45 deaths.

Back in the US of A, the annual round of county fairs are back in swing.

And, naturally the H3N2v influenza virus has emerged again as has been totally predictable.

This next article outlines the current situation, and of course there are a few pertinent observations that arise for this article.

Firstly, the obvious question.

Are exibitors required to provide up to date vetenary certificates of the health of the animals they intend displaying at these county fairs, surely this would be a minimum requirement and expectation.

The next is to note that it would appear that this article seems to be a little up-beat that there have been no hospital admissions from the virus so far this year, and this is indeed good news.

But, what all this missplaced optimism neglectes to observe even though the article itself carries the information, is the fact that this virus carries the H1N1 Pdm9 "M" gene which it has aquired by reassortment with the H1N1 influenza virus and as a result is now significantly more contagous than ever.

How this virus aquired this "M" gene remains unexplained and continues to raise serious questions, but aside from its origins, this new H3N2v virus is now far more likely to spread very readily throughout the human population and sooner or later it will find the opportunity to again reassort its genetic make up with ultimately a highly deadly form of influenza, perhaps H5N1 avian influenza, and remove its final impediment to becoming an historical pandemic capable of destroying around 60 percent of the human population of the world in its first wave as it spreads across the globe.

It is inevidible that sooner or later this will happen, nature alone will guarantee that, but with a guiding hand along the way that inevitible pandemic will commence years sooner if indeed this is what has beenis motion as I strongly suspect of recent times.

Certainly there are groups among us who would see such a depopulating pandemic as desirable, and there have been very clear demonstrations of that type of goings on having already happened.

This Health Alert Network Health Advisory provides an update on H3N2 variant virus (or "H3N2v") activity and summarizes CDC's updated H3N2v case definitions and recommendations for H3N2v surveillance for the summer and fall of 2013. It supersedes the last H3N2v-related HAN Health Advisory, HAN 325, which was issued 3 Aug 2012.

Background
----------
The 1st cases of influenza A (H3N2) variant1 (H3N2v) virus infection this year were reported in June 2013. These cases were associated with exposure to swine at an agricultural fair prior to illness onset. H3N2v viruses with the matrix (M) gene from the 2009 H1N1 pandemic virus were 1st detected in people in 2011 and were responsible for a multi-state outbreak [in the US] in the summer of 2012 that resulted in 306 cases, including 16 hospitalizations and one fatality. Genetic sequencing by CDC has confirmed that H3N2v viruses isolated in June 2013 are nearly identical to those detected during summer 2012. Most cases of H3N2v identified during 2012 were associated with exposure to pigs at agricultural fairs. Agricultural fairs take place across the United States every year, primarily during the summer months and into early fall. Many fairs have swine barns, where pigs from different places come in close contact with each other and with people. These venues may allow spread of influenza viruses both among pigs and between pigs and people. Data indicate that infected pigs may spread influenza viruses even if they are not symptomatic (e.g., coughing and/or sneezing). Although instances of limited person-to-person spread of this virus have been identified in the past, sustained or community-wide transmission of H3N2v has not occurred.

Clinical characteristics of the 2012 and 2013 H3N2v cases have been generally consistent with those of seasonal influenza, and have included fever, cough, pharyngitis, myalgia, and headache. Of the 16 H3N2v hospitalized patients, most were at increased risk for complications of influenza because of age or the presence of an underlying medical condition. None of the persons ill with 2013 H3N2v infection have been hospitalized, and no deaths have occurred among them.

Bird flu (H5N1) virus has once again been seen in the capital. The deadly H5N1 strain was confirmed at a Gothathar-based farm.

The contagion was confirmed on Monday [1 Jul 2013] after an investigation of the layer chickens at the poultry farm. A sample from the chickens was sent to the Central Veterinary Laboratory after they started dying last week. Experts have been sent to destroy the chickens.

According to District Veterinary Office chief Dr. Bolraj Acharya, out of the 2000 chickens on the farm, 600 have already been culled, and the rest are also being killed. Acharya further stated that the District Veterinary Office is working on more strategies to stop the epidemic.

A 6-year-old girl from Cambodia's southwestern Kampot province has died of avian influenza H5N1, bringing the death toll to 9 and the number of cases to 13 so far this year [2013], a joint statement by the World Health Organization and the Cambodian Health Ministry said Tuesday [2 Jul 2013].

The girl was confirmed positive for human H5N1 avian influenza on Friday last week [28 Jun 2013] by L'Institut Pasteur du Cambodge, the statement said, adding that she developed fever and headache last Monday [24 Jun 2013], and her condition worsened with cough and dyspnea. The girl was taken to a private clinic until, last Friday, she was admitted to the Kantha Bopha Hospital with fever, dyspnea and cough, it said. "In the Kantha Bopha Hospital, the girl was treated with Tamiflu but died later in the night," it said. "There have been recent poultry deaths in the girl's village, and the girl was likely exposed to sick and dead poultry before she became sick."

So, now even history holds the unequivical proof.

Unfortunately, attendance at this years Hajj by religous pilgrims will without question be putting the worlds population at serious risk.

This study both reinforces this assertion but also confirms by scientific testing tigor that these risks are a clear, present and approaching danger to the humanity of the world.

And the World Health Organisation in co-operation with, or blackmailed by, the Saudi Health Officials have permitted this situation to develop to today where it can no longer be slowed, stopped or prevented.

Circulation of Respiratory Viruses Among Pilgrims During the 2012 Hajj Pilgrimage

Abstract: The Hajj is the oldest and largest annual mass gathering in the world and may increase the risk of spread of respiratory viruses. We performed a prospective survey among a cohort of pilgrims departing from Marseille, France to Mecca in the Kingdom of Saudi Arabia (KSA) for the 2012 Hajj season. Nasal swabs were collected from participants and tested for 11 respiratory viruses by real-time reverse transcription polymerase chain reaction.

Of 165 participants sampled before departing to the KSA, 8 (4.8 percent) were positive for at least one virus (5 rhinovirus, one influenza C, one adenovirus, and one enterovirus). 70 symptomatic pilgrims underwent additional nasal swabs during their pilgrimage in the KSA, of which 27 (38.6 percent) were positive for at least one virus (19 rhinovirus, 6 influenza A, one influenza C, one respiratory syncytial virus (RSV) B, one metapneumovirus, one adenovirus, and one enterovirus). This was significantly higher than the 4.8 percent who were positive before departing for the KSA (P less than .001). Of 154 pilgrims sampled before leaving the KSA, 17 (11 percent) were positive for at least one virus (13 rhinovirus, 3 adenovirus, 2 influenza B, and one enterovirus), which was also significantly higher than the percentage of positive pilgrims (4.8 percent), before departing for the KSA (P = .040).

This study suggests a rapid acquisition of respiratory viruses among pilgrims during their stay in the KSA, most notably rhinovirus, and highlights the potential of spreading these infections in the pilgrims' home countries upon their return.

There have been 2 outbreaks of H5N1 highly pathogenic avian influenza (HPAI) on poultry farms in the central state of Chhattisgarh.

The Indian veterinary authority sent an immediate notification dated 5 Aug 2013 to the World Organisation for Animal Health (OIE).

The report describes 2 outbreaks of HPAI at farms in the central Indian state of Chhattisgarh, both starting on 30 Jul 2013.

Of the 2149-strong flock at Anjora, the poultry production unit at the College of Veterinary Sciences and Animal Husbandry in Durg, 464 birds died.

At a government poultry farm in Jagadalpur, there were 3600 affected birds including 2392 deaths in the flock of 6293.

After some considerable time, there has been a re-emergence of H7N9 influenza again in China.

This fresh case has appeared much closer to Hong Kong than previous cases and could pose a serious threat that if this deadly influenza strain again re-commences to spread through human populations, it could easily spread to Hong Kong where it would have devastating consequences.

As Hong Kong remains a significant travel hub for the region, there must be serious concerns if this bug manages to migrate there and gain any sort of significant foothold in the general population there, as this would provide the perfect opportunity for this bug to spread itself internationally in very time frames, making it almost if not completely unstoppable.

Hong Kong heath authorities said on Friday [9 Aug 2013] they were monitoring avian flu developments on the mainland after a woman in Guangdong [province] tested positive for the new H7N9 strain. The case involves a 51-year-old woman living in Huizhou, about 100 kilometres [about 62 miles] northeast of Hong Kong, who had contact with poultry before falling ill.

The woman had been tested positive for the H7N9 virus in Guangdong provincial laboratories, but her samples were being sent to Beijing for further tests to confirm the result, said Hong Kong Centre for Health Protection controller Dr Leung Ting-hung. Leung also said Guangdong authorities notified the centre of the case on Friday [9 Aug 2013]. In view of the large number of H7N9 cases in eastern China and the significant mortality rate among them, we must not underestimate the risk to our local citizens, said Dr Leung Ting-hung. The woman had contact with 36 people, none of whom have showed any bird flu symptoms, Leung said, citing information provided by Guangdong authorities.

The new H7N9 was 1st identified in eastern China, with the 1st known human case of the virus reported in March [2013]. So far, 134 cases have been reported, resulting in 43 deaths. Leung said that given the busy traffic between Hong Kong and the mainland, it was possible that the virus could spread into the city.

Understanding the potential role of animals in the emergence and spread of Middle Eastern respiratory syndrome (MERS) requires further investigation, analysis and study, FAO said today [9 Aug 2013]. Current evidence is not sufficient to identify the specific source, whether animal or otherwise, of the coronavirus that is causing MERS in humans.

Coronaviruses are a family of viruses that affect primarily birds and mammals. Some strains cause mild disease, while a limited number are more harmful (e.g. the severe acute respiratory syndrome [SARS] coronavirus). The MERS coronavirus has been shown to cause acute respiratory illness in humans. It has not yet been shown to cause disease in animals.

"It is not yet clear how people are becoming infected or where the virus might come from," stated Juan Lubroth, FAO Chief Veterinary Officer. "We do not have enough information to identify with certainty the virus's origin. Confirming the source and mechanisms of transmission and spread are key to developing ways to reduce the risks posed by this virus to humans or other countries."

A study led by the National Institute for Public Health and the Environment of the Netherlands and just published in The Lancet Infectious Diseases Journal provides some additional information [details in 20130809.1872008].

The study found antibodies for the MERS coronavirus or a similar virus in camel blood samples. The samples were taken in areas where human cases have not been reported. In some cases, the tested camels have been isolated from other camels for many years.

These antibody findings indicate that the MERS virus, or a similar coronavirus, occurs in some camels and potentially other species. However, the only way to know with any certainty whether the virus affecting humans is the same as the virus possibly affecting camels (or any other animal) is to isolate the virus in different species and compare them genetically.

To date, the MERS coronavirus has only been isolated in humans. Investigation and research in animal species must continue in order to shed light on potential animal sources. If and once identified, veterinary and public health authorities could better communicate how to prevent infection or institute specific control measures.

Is this a simple case of paranoia?

At first glance one could be forgiven for thinking so, but then when those voicing these suspicions are people of their calibre, then some weight must be awarded to their assertions.

Also, not withstanding the obvious, there have been previous clear examples of the deliberate type sabotage being spoken of here to warrant considerable caution and care before simply jumping to simple and uncomplicated answers.

At the very least and even in the complete absence of any paranoia, all due dilligence is should alwys be the baseline standards from which to work from.

NO second party should ever place its blind trust into the integrity of those why provide what they claim to be safe products.

I definately endorse and condone any actions that cross-check and confirm the reliability all these issues, as simply we cant afford to be making mistakes, let along be used as lab rats.

Tulsiram Dhukkhwa, chairman of the Poultry Farmers’ Association (Bhaktapur), said many poultry farmers in the district have complained that bird flu broke out in the district because of unauthorised use of vaccines. “Unauthorised vaccines are responsible for the bird flu outbreak in Kathmandu and Bhaktapur,” he said.

He said vaccines must have caused outbreak of bird flu in many places all at once. Dhukkhwa said powerful countries have been using poor countries like Nepal as a laboratory. “If Nepal has been used to test vaccine developed against bird flu in the USA, it is a misfortune,” he said. Dhukkhwa claimed that samples of dead birds have already reached America.

Two more new human cases of avian influenza have been confirmed for the H5N1 virus in Cambodia, bringing the number of cases to 16 so far this year [2013], a joint statement by the World Health Organization and the Cambodian Health Ministry said on Tuesday [13 Aug 2013].

The statement said that one case is a 9 year old boy from north western Battambang Province, who was admitted to the Jayavarman VII Hospital in Siem Reap with fever, cough, vomiting, abdominal pain, and dyspnea [difficulty breathing] on 4 Aug 2013, and was confirmed positive for human H5N1 avian influenza on Friday last week [9 Aug 2013]. "The boy is currently in a stable condition," the statement said.

The other case is a 5 year old girl from Southern Kandal Province who was admitted to Kantha Bopha Hospital in Phnom Penh on Friday last week [9 Aug 2013] with fever, cough, sore throat, abdominal pain, diarrhea, and dyspnea, and she was confirmed positive for human H5N1 avian influenza a day later. "The girl was treated with Tamiflu and is currently in a critical condition," it said.

Its just good advice to never let your guard down especially when you are dealing with our primary focus on this thread.

Recently there hasnt been a lot of significant new news to report and I admit to taking a step back from the commitment to report every crumb of information that surfaces.

However, I still do regularly check my primary sources for fresh developments and today this pops up.

While at this point in time, of itself, this is perhaps not too noteworthy other than to be aware that a new strain of influenza virus has been detected and now should be added to our watch list just an case something significant does develop.

As usual, the largest potential risk to humans will most likely be that should this strain manage to reassort itself with a more virulent strain that is active and already presenting some concerns, we could find ourselves having to deal with yet another threat in conjunction with the various others that are more or less active in various regions around the globe in these times.

It does 'feel' like the numbers of circulating strains are becoming more prolific as well as diversified, but that might just be missconception from focussing so heavily on the specific topic.

Time will tell, as always.

University of Hong Kong microbiologists have discovered a new strain of influenza virus in dead pigs at a slaughterhouse in Sheung Shui. The team has named the virus porcine parainfluenza virus 1 (PPIV-1). The swine virus is not expected to pose a significant health risk to humans as long as the pork they eat is well cooked. But a top researcher behind the study, microbiologist Yuen Kwok-yung, warned it might mutate and jump from pigs to humans.

There are some very interresting observations being conveyed in this commentary regarding the evolution of H3N2, that are again being associated with the annual round of county fairs in the US.

For those who followed that saga last year, these sequences and the facts that they reveal again pose more questions than they answer, especially when placed against those very vexing questions that were originally raised last year.

This bug is just not behaving in the manner that it should be expected to display, again leaving annoying and unresolved questions that lie outside what is normal.

We shall wait and see what the good Professor has to further say about this topic as he gains greater quantities of information.

The only Ohio resident with confirmed H3N2 in 2013 had visited a agricultural fair in northwestern Virginia just prior to disease onset.

Similarly no cases have been reported in Minnesota this year although the July 11 isolate, A/swine/Minnesota/A01351603/2013, also matched 2012 human cases.

In contrast to the frequent detection of 2013 swine matches with 2012 human cases, only one swine sequence, A/swine/Illinois/A01432795/2013, isolated in June is closely related to the 2013 human cases, which all have a closely related H3 which has either N145K or K145R (the Illinois swine has N145K). Thus, there is significant discordance between swine sequences, where there is just one closely related to the human sequences, while all 16 human cases in 2012 are closely related even though they are linked to fairs and farms in Indiana, Virginia, and Illinois.

Moreover, the three most recent human cases (A/Ohio/05/2013, A/Illinois/03/2013, and A/Indiana/11/2013) all have an H1N1pdm09 PA gene segment (as well as an M gene segments) which is identical in the isolates from Illinois and Indiana and are virtually identical to the sequence from Virginia (diagnosed and confirmed in Ohio).

This discordance again suggests that humans are infecting swine, which is why the human sequences share changes that are rarely found in swine collections made prior to the fair season.

At first glance this next article on the possible vector for H7N9 influenza that caused many infections and deaths earlier this year in China, struck me as pretty unlikely.

In fact even now, on reflection, to a degree it still does.

There are simply too many other what ifs, that this current proposal seems to be overlooking.

The outstanding fact that this particular virus does remain so detectable in such a high percentage of positive cases in their faeces, and that this is an almost unique characteristic not normally observed with other influenza viruses, is naturally very significant.

But this theory simply doesent explain the huge geographic jumps that the bug made overnight nor does it answer other factors like how it sustained transmissibility when wet poultry markets were closed.

The list goes on, and while the statistics are singly very compelling, the other open ends cant seem to tie in with the proposal.

Well at least in my opinion.

As I pointed out much earlier, Id be looking at the food sources for the poultry once they reached those wet markets, as opposed to the commercial foods that the large and unaffected commercial operations were using.

H7N9 bird flu may be spread through human faeces

The H7N9 bird flu that emerged on the mainland in March [2013] may be spreading through human faeces, as university researchers found the virus in the stools of 4 out of 6 people killed by the [virus] in Zhejiang. Theoretically, the virus could spread in the same way as severe acute respiratory syndrome (SARS) did during the 2003 outbreak in Amoy Gardens, Kowloon Bay -- through sewage pipes -- University of Hong Kong microbiologist Yuen Kwok-yung, a top researcher behind the cross-border study, told the South China Morning Post.

"This has important implications for infection control strategies for H7N9 virus infection, as influenza virus in stools may contaminate the surrounding environment," Yuen and his team reported. But the team also suggested that the fact the virus may be transmitted via faeces was the reason why, so far, few cases of human-to-human infection had been recorded, Yuen said. Hong Kong University conducted the joint study with Zhejiang University's First Affiliated Hospital in Hangzhou and published the results in the Clinical Infectious Diseases journal on Tuesday [13 Aug 2013, but not yet available]. They tested 12 patients who were admitted to the intensive care unit between 10 and 23 Apr 2013, half of whom have died.

During postmortem tests, stool samples collected from 4 of the 6 fatalities returned positive results for the H7N9 strain. The rate -- 67 per cent -- was much higher compared with those found for human [seasonal] flu or H5N1 bird flu, which ranged from 5 per cent to 33 per cent, the report said. None of the blood or urine samples from the 6 dead patients carried H7N9.

This has got to be a bit of a red herring.

There are now reports of three cats (domestic?...unstated)that have apparently died from the affects of Saudi SARS.

Really this isnt very significant news, H5N1 avian influenza is a known killer of cats in Egypt where they are quite prevelant in the city areas.

As mammals, and often sharing human living space, this situation is only surprising in that it hasnt been previously reported.

Any suggestion at this late time that these animals might be the reservour would be rediculous beyond comprehension, though I wouldnt put an attempt by either the World Health Organization, nor the Saudi health authorities for attempting such a stunt, given their performance to date in dealing with this deadly pandemic potential.

According to the newspaper "Middle," the director of the clinic, Dr Khalid Ambassav explained that cats died for their inability to resist the virus. They suddenly stopped eating, had high body temperature above 40 deg C [104 deg F], "increased fluid in the cat's body, bulging stomach, lack of balance and inability to walk, and cirrhosis of the liver, and destroyed kidneys", resulting in the death in a period not exceeding

The Federal Government will today announce parents who choose not to have their children vaccinated will miss out on thousands of dollars in government benefits.

Prime Minister Kevin Rudd will announce that those who do not vaccinate their children will not get the Family Tax Benefit A end-of-year supplement.

The payment is worth $726 per child, per year and is paid when children are vaccinated at one, two and five years of age.

Since last year, parents who have not immunised their children have not received the benefit; however, those registering as so-called conscientious objectors have.

Under Labor's policy, exemptions would only be made on religious or medical grounds.

Labor says it wants to boost immunisation rates and prevent children who are not vaccinated from getting diseases like whooping cough and measles, and putting others at risk.

The Government says the science of immunisation "cannot be disputed".

My heart goes out to those parents. With the issue of vaccination, I thank my lucky stars on a very regular basis. I had severe allergies as child, vaccines among them. I have medical grounds for refusal when it comes to my daughter, although I do have to file an exemption yearly with the school board. Even in Canada, contentious objectors are sometimes given a difficult time.

There can be no room to tollerate any government assuming blanket policy that ignores the sacrosanct right of a parent and their responsibilities in rearing their podgeny.

Educate by all means, but outright lying to push some poorly founded belief system it inexcusable in the highest order.

Anybody would think that they are talking about bubonic plague.

Thanks for your comment, but to be honest it does seem that nobody is interrested in supporting this thread unless I unput at length on very regular intervals, which I have willingly done till now.

I do check in regularly though and I am just a bit dissapointed that I seem to be the main contributor which was never the intent.

I hope that that might change.

Here's hoping as there is a great deal to update and report on, but I simply dont want to be the one who has to shoulder the complete load, apart from the fact that my view alone gives a slanted view, no matter how I might try to avoid that.

Sometimes I do intend to be opinionated and very challenging when the situation requires, but that still denies good balance, so I appeal to everyone to come forward and to add your commentary and views in the interest of presenting a balanced forum on a topic that time has proven to be of value.

Im still here and will continue to monitor activity, but Im simply not prepared to devote up to 5 hours a day without appropriate support.

Sorry if my limits have to be stated in this way.
