China is missing in your list. Their governments were more keen on killing their own people but still if number of deaths and ruthlessness are to be considered, then China is my candidate.

I also think the true numbers from Cultural Revolution will never be released.

Also Russia sold food to nazi Germany, while their own people starved by millions. Ukraine - the best soil in Europe, couldn't feed themselves and millions died.

I guess it depends on criteria you choose.
