The disease IS confirmed by brain biopsy performed in the autopsy room. In my opinion, most cases are caused by contaminated surgical instruments. The second person has the symptoms; they won't be able to confirm it until autopsy. It is pretty easy to diagnose - nothing else looks like it. I've seen about 5 cases in 33 years. There is a national prion disease laboratory where the brain specimens are sent - they confirm the presence of prions.
