The guy must have been either drunk or loaded on something to the point where he didn't feel or the pain didn't register. Otherwise any guy in danger of loosing his testicles would react with necessary violence to get whomever off of his balls. A punch or a kick or if necessary, smashing any nearby object into the side of their head. I know just thinking about such a thing happening and all kinds of methods occur to me to stop such an attack. Up to and including those which might be deadly to the attacker. (Like at that point do I care if they're injured or even killed?)

Maybe he deserved it....
