Once I had a mild case of flu for which I took oil of oregano, two drops under the tongue (yeow!) about 4 times a day. I was surprised when some little chunks of plaque just fell off my teeth after about the third day. I had to spit them out. It was completely unexpected. And yes, it WAS plaque.

Another thing you could try, get one of those hokey looking gum massagers. They look like a short metal chopstick with a red rubber tip that you use like a toothpick - drugstores carry them. First brush your teeth, then floss and rinse. Think your teeth are clean? Think again...use that little rubber point all around the gum line and in between the teeth. Use it like a toothpick and you will be AMAZED at what brushing and flossing missed. Once I started using that thing, I am hooked, as it gets your mouth SO CLEAN.
