The "problem" of HIV transmission is that HIV positive people who are on medication have a 97% reduced risk of transmitting the disease. Homophobia contributes to people fearful of being tested and treated which would reveal their identities as gay. Many of the straight and bisexual HIV positive people are afraid of their spouses and girlfriends finding out about their secret life. If all the HIV positive people were in treatment the epidemic would swiftly end, since in treatment there is almost no transmission of the virus.
