If you're just out of shape I would jump directly into the Insanity program topped off with applicable martial arts such as Jiu-Jitsu,Muay Thai,Aikido, and Krav Maga.

If you're in no shape at all and could be at risk of heart attack, stroke or low blood sugar issues you should start with a good cardio workout and a little resistance training and make sure to get your blood pressure checked out. I won't lie to you. You're probably dead but who knows? Gotta be thinner than the next fat dude I guess. Good luck.

Knife throwing and hand-to-hand is amazing to have as a tool as well as bow skills. I'm obviously speaking in terms of simple survival in case of not being prepared especially with weapons and self-defense. LEARN HOW TO USE A STICK/KALI/ESCRIMA/BATON!!! Sticks are everywhere. Learn how to use it.

buy a heavy bag and USE IT. it will do no good just hanging around. hit that thing like your life depends on it.

punches,knees,elbows hit it till you friggin bleed. you will get in shape.

if you dont know the proper techniques for punching i'm sure there are 1000's of you tube videos to help.
