Regarding who filmed the lift off, I Googled it and found this answer, second link down:

The external pictures of the LM blasting off from the lunar surface were taken by video cameras on the lunar rovers, which were left behind on the Moon. The cameras were operated by remote control from Earth. On Apollos 11, 12, and 14, on which no rovers were taken to the Moon, there were no pictures taken of the lunar surface ascent.

Hmmm. That's pretty good camera work from Earth, don't you think? Filmed via remote control? How long does it take for a radio signal to reach the moon from Earth? Fast enough for a perfect pan as the lunar module lifted off, never losing it? Or maybe the speed was just calculated ahead of time and all they had to do on Earth was hit a button to initiate the pre-programmed pan. But I seriously doubt it.
