Say a hip replacement surgery will improve your QoL by 1.5 points on a 10 point scale and you have a life expectancy of 12 years.

This means your treatment has a value of 1.5 * 12 or 18 points. Any treatments with a higher score will be paid first. If money is left, you get your treatment. Otherwise, you don't.

Budget cuts, old age, and expensive treatments relative to the benefit the government decides it confers all work against you.
