Talking to himself'

I know this man who talks to himself in his head and then answers the questions aloud. Sometimes his conversation will continue aloud and then it will stop(I think he continues it in his head) mid sentence. He does this when there are clearly people who can hear him around.
My question is does anyone know what this could be a symptom of? He is a white male, 55.
