And since her birth name has been reported as Gilberte Khawam, there must be a record of an official name change from Gilberte to Jill. She signed her legal documents as Jill so that must be her legal name.

Somehow I doubt there is any record of this name change either...

It's never been clearly identified whether she was born in Lebanon or here in the U.S...I find it suspicious that it's always said that her parents fled Lebanon in the "mid-70s". Well "Jill" was born around 1975 so mid-70s isn't really specific enough to know one way or the other.
