I always took it to be a deep conversation between Bowie and himself, in which he was arguing a specific point.

If I got the correct meaning, then on some level, when he wrote the song, he felt that he was a bit of a sham. A common feeling for the wildly successful, since they all know on some level that their work isn't that much better than those that are also good, but who don't make it.

He was showing a conversation between that "dark" part of himself that "Sold the World" on the idea that he was worth something. The "real" part of him had thought that part was dead and gone, but there he was.

"I thought that you died long ago..."

"Oh, no, not me. I never lost control..."

Just one take on it. A lot of people think it means something very different and Bowie hasn't said, as far as I know.
