norespect And if you are an ER doc...you get no respect, almost none.

 Quoting: allisaTX

And April 15th is a joy. I/3 of your paycheck goes to Uncle Sam. And then you have the hell of worrying if you underpaid during the year.

It's just a grand!
 Quoting: allisaTX

Why do you get no respect as an ER doc?
 Quoting: Jervis and V-Honey

I believe it is because ER docs are seeing people at their worst. See patients no longer are respectful of their doctors in general. This is a level one trauma center. Patients demand antibiotics, they demand plastic surgeons for a small laceration, they demand pregnancy tests, etc. etc. And they demand narcotics. Many are thankful and understand, but not many.

Recently my husband had a patient or a "client" as they are asked to call them say to him..."Oh I remember you, you waited on me before"....ok....that feels like crap to an MD....call us elitist or whatever, but "you waited on me before?"

And the sad thing is he has lost the understanding that this is a noble life he is living...a noble profession...he has saved many lives, but he has had no life of his own..

we are separated..
 Quoting: allisaTX

Oh i gotcha. Ya, u r in a tough dept indeed. I took it that you were not garnering respect from your fellow clinicians/providers.....i was like really?!

Anyway, yes....you are in the trenches when in the ED, and people will behave at their worst. hf to you.

Nope, the nurses to the dirty work and that's what they are paid to do...My sister is an RN, I have many friends and clients that are doctors and dentists, they hire nurses and hygienists, to do a particular job, it's reasonable and it's fair...
 Quoting: BRIEF

Yes, it is true the PA's etc. follow the orders, but the MD is the one the lawyers go after. Plus if you didn't didn't come from money, and did it all on loans, you were looking at about 200K in debt after residency if you had a kid or two along the way.

I know. I lived it. And we are not as well off as you would think. Not if you are an ER doc. It was too big a price to pay if you want the truth, at least for our family .
 Quoting: allisaTX

A friend of mine and his wife (met in dental school) are 40 and a million dollars in debt...that's $1,000,000.00 plus interest for loans on school, house, car, and buying a small practice...plus interest...if they make 300K a year, Obama considers them rich, but that barely puts food on the table with all their payments and hiring a couple of helping hands...
 Quoting: BRIEF

Wow, that's sad, because that puts them on the treadmill right away. Part of the problem is doctors are not very wise business people. We had been poor so long, we only took out bare minimum loans for living. So we lived above our means from the beginning instead of paying off loans and living modestly, I thought I should "live" a certain way. It was partly because we were in Dallas and it was such a different lifestyle from what the average northeasterner was used to living, especially back in the early 90's.
 Quoting: allisaTX

I tried to counsel them, I'm a financial counselor, but they were sold on the banker that lectured them on "good debt" as though it was a blessing for the bank to enslave them...they bought it hook, line, and sinker and took the loans :(

LOL, no...I know doctors, and dentists, and no Nurse has the training, skill, or knowledge they have...or they would be doctors!

Yeah true, but youd be surprised at how many do their job.

Probably a million reasons.....but.....some of them never intended to want the physicians job....the expectations of them just go that way. This is true, and really isnt funny at all... You would be surprised if you only knew. Part of this massive electronic provider order entry conversion that is happening across the US, has many a doc frustrated. It forces them to write their own orders.....which a lot times, doesnt happen now. This leads to all kinds of problems and can lead to the death of a patient. Writing orders is practicing medicine. To have a non provider do it, is wrong. But there hasnt been an electronic trail till now. That is a huge frustration. Its not like the nurse asks to do the docs job, it just gets expected of them.

Now, to the defense of a doc, it is also frustrating, they dont all get paid the way they should either. And now, so much more is expected of them. Especially if they admit inpatients to several hospitals, think of all the systems they have to learn....

Its tough in the hospital world. Really tough.

Oh that makes my heart sink. If only I had been wiser and educated myself about money. I was a technician, working class background, I had no understanding of debt. Now I do of course and would give anything to do it again. The private school, the huge house, the piano lessons, the cars, I did it for my kids thinking that was right, but I didn't understand how ist was hurting my kids that we were not happy....then came the psychiatrists for the kids and the more expensive schools for the kids and the 1000 week psychiatrist bill for all of us. BUT EVERYONE was living that way. So many of the kids in the neighborhood were on ritalin or addeall and we just took it.

We were a tragic bunch and they still are down there in the park cities in Dallas. It's hell.

Many in the Park Cities in Dallas are happy and content, my son was able to assimilate but I could never adjust to that lifestyle. He says they network and have a great life, but it was not a life my soon to be former husband and I should ever have tried to live.

And I apologize for TMI, I've had a bit of wine and this is a subject that hits too close to home, so I'll be quiet now.

Thanks for listening.

LISTEN UP. I WANT PEOPLE ON THIS THEAD TO HEAR THIS.

OUR HEATH CARE PREMIUM IS 1, 800 per month.

Because he works for EmCare Inc. and they do not have a group plan for their doctors.

So an ER docs family pays that and has a 10k deductible. We can't afford to go to the doctor.

What do we do drop our damn individual insurance?

I have tried hard to find an association that will let us in on a group policy. The AMA policy pays for everything BUT psychiatric care...and that's what we need it seems.

THey also do not pay if you are in an accident and their are drugs or alcohol involved.

Do you see how this is a two tierd system? One for the poor and one for the "rich"....we are in the damn crack and we gave all our money through taxes and student loans to the government. Meanwhile...the sweat of the docs backs are making EmCare Inc. rich as hell!
rant

P.S. Oh what a blessing we can now keep our "children" on the 60 yo docs policy until they are 27. Thanks Obummer. Nice one.

Yes, it does, it is a fee for service deal, but very few doctors abuse that code issue. The majority of physicians were brought up to be honest, caring, good citizens. At lease on the ER end that has been what I have seen. I was an ophthalmology tech. now there my experience was not quite the same.

Also they don't get the coding issue. It's always brought up to them in a monthly meeting and they have someone who checks their charts to see if they are charging the maximum they can.

And each month a chart is sent to all the ER docs and it is made very clear to them if they are not up to the speed they would like them to be.

Here's the truth:

If the doctors are having a hard time we are doomed.

Really.

And I don't have to tell anybody why.

They are not going to pay nurses that salary and nurses or PA's are not going to multiply the money like the doctors have.

Nurses and PA's don't hire landscape architects or interior designers to my knowledge. Maybe they just pay less.

So everyones pay will go down from here, unless I am missing something.

Hospitals are about profits, not health care. Its been that way for a while. Is this good medical practice?

[link to www.publicintegrity.org]

The Texas hospital’s billing pattern is far from unique. Between 2001 and 2008, hospitals across the country dramatically increased their Medicare billing for emergency room care, adding more than $1 billion to the cost of the program to taxpayers, a Center investigation has found. The fees are based on a system of billing codes — so-called evaluation and management codes — that makes higher payments for treatments that require more time and resources.

Use of the top two most expensive codes for emergency room care nationwide nearly doubled, from 25 percent to 45 percent of all claims, during the time period examined. In many cases, these claims were not for treating patients with life-threatening injuries. Instead, the claims the Center analyzed included only patients who were sent home from the emergency room without being admitted to the hospital. Often, they were treated for seemingly minor injuries and complaints.
