Has anyone EVER seen the USGS raise an earthquake, instead of downgrading every single quake?

Yes!!! I've seen USGS raise the magnitude of an earthquake. I forget which one, though, but it has happened and it was definitely other than the scale change you mentioned. It's rare, but they have.
