I have gone through this with my mum, who passed away at age 99 in 2010.
My mum suffered TIA's (Trans Ischemic Attacks) (or) small strokes. When she had a TIA she acted strange and thought she was in the past.
The other thing that made her act that way was urine infections. I called the doc out one time, convinced she had suffered a stroke. Doc arrived and asked her who the Prime Minister was and she replied 'McMillan' He asked her what her DOB was and she answered correctly right off.
Urine infections in older folk are very common and cause confusion and strange behaviour.

Yes, UTI our other infection, TIAs/stroke, and vitamin/mineral deficiencies/imbalances are the first things that should be checked for, as they can cause similar symptoms and are more treatable.
