So all the "redundantly powered" servers are dead. If some on either A or B used circuit C as their second power instead it can go too, and so on. Variations: the computers were already using more than the battery back up or backup utility or generator could provide. Why? Someone was cheap. They installed too many servers for the power. Cheap penny pinching manager decided backup power or backup servers wouldn't have to be as powerful or better than the main ones - it's just the backup, after all, it isn't even going to be running!
