I once knew of a corp. employee (in position of financial responsibility/budget) that paid for and purchased thousands of dollars in prepaid Walmart gift cards to give out as incentive for a very large team/project going on.

Come to find out, this individual (had a gambling problem) (allegedly, cough cough) kept most of them (and spent on self) and could never produce a listing of names of whom received them.

There was a witness (a contractor that ended up terminated for a made up excuse, when in reality it was due to seeing the "wrong" things). Said allegations against individual were never fully investigated (pays to be on personal terms with a CEO). Slap on hands for not producing listing of whom cards were given too; yet those cards could have been tracked by their issue numbers. They weren't, was swept under the rug. Person was also in a protected group, so, you know how that goes: Hands off while cookie jar is being raided in broad day light.

Accountants can be very creative and those in power can shut down an investigation so that it would never see the light of day.
