Wouldn't make any sense. Think of that like saying:

"Well what if one of the Ferguson police officers is actually a doctor because somebody has medical issues... So they disguise the doctor as a police officer..."

It makes no sense. EMS is always readily available and if you think the above scenario sounds silly enough, just replace Ferguson police department with the most elite police guard duty, th secret service, and see how much sense it makes. A cohesive team that trains constantly. They wouldn't put a civilian doctor in one of their outfits and then put him in situations that he could not perform their duties in.

A doctor wouldn't be close enough to a candidate to cause harm, let alone in a cluster of secret service agents. It wouldn't make any sense to have a civilian doctor dressed like a SS agent with a SS lapel on. There would be something to distinguish that the individual was a doctor so that when he says "Hey, I'm a doctor!!" Nobody thinks he is just a SS agent.
