WASHINGTON (AP) - Air Force officials say three ballistic missile crew members in North Dakota fell asleep while holding classified launch code devices earlier this month, triggering an investigation by military and National Security Agency experts.
The probe found that the missile launch codes were not compromised, but the incident comes on the heels of a series of missteps by Air Force that has put the service under intense scrutiny.
Officials called the incident a "procedural violation," and said the Minot Air Force-based crew had code devices that were no longer usable, since new codes had been installed in the missiles.

Thats the million dollar question and I can only speculate untill there is an event. Maybe a nuke gets used in a "terrorist" attack and it gets traced back the military. Who knows maybe a nuke is stolen. All speculation though till there is an actual event. |

CNN’s Barbara Starr reported that the Air Force said that a crew in charge of outdated nuclear missile launch codes fell asleep while on duty.
Air Force Times reported:

Minot airmen fell asleep with classified nuke hardware

Three missile officers with the 91st Space Wing at Minot Air Force Base, N.D., fell asleep July 12 while holding a classified “code component” — a hardware device containing the codes needed to activate the control system for Minot’s intercontinental ballistic missiles.
The device, kept in a locked case, installs the codes that allow the missile launch control center to command the ICBMs in the missile silos. One piece of hardware is installed and the old one is removed, somewhat akin to changing hard drives in a computer.
Four officers had completed the process of changing the codes for the system underground in the launch control center and had returned topside to the large living center, which looks much like a ranch house inside and includes six bedrooms, a large kitchen and dining area, gym and a security control center for security forces airmen, according to Air Force officials.
While waiting for permission to bring the device back to base, the three officers with the code component in their possession fell asleep. The fourth officer was not present.
When they woke up, the officers reported the incident to their command, said Col. Dewey Ford, a Space Command spokesman. Representatives from U.S. Strategic Command, Space Command, the 91st Missile Wing at Minot, the 20th Air Force — headquartered at F.E. Warren Air Force Base, Wyo., and the National Security Agency investigated the incident. They found that the missile launch codes were not compromised.
Ford called the incident a “procedural violation,” and emphasized that the code devices were no longer usable since the new codes had already been installed in the missiles. Ford said public safety was never at risk.
Defense Secretary Robert Gates was not notified.
“Because [the codes] were never compromised, [the Air Force] did not contact top levels of leadership,” Ford said. “The procedural error involved the fact that they fell asleep. If they had stayed awake, this would not have been an issue.”
Asked for comment about the incident, a Senate Armed Services Committee staffer said senators had not been informed.
During their confirmation hearing before the committee July 22, Michael Donley, who’s been nominated to become Air Force secretary, and Gen. Norton Schwartz, the nominee for chief of staff, said their top priority would be shoring up nuclear surety and restoring the country’s trust in the service’s ability to manage its nuclear weapons and mission. They did not mention the incident.
Minot has been ground zero in the ongoing crisis surrounding the Air Force’s inability to properly manage and secure its nuclear weapons.
Last August, airmen of Minot’s 5th Bomb Wing mistakenly loaded six nuclear warheads onto a B-52, which then flew to Barksdale Air Force Base, La. The Stratofortress then sat on the runway at Barksdale for more than 12 hours before anyone noticed that the bombs under its wing, which were supposed to be dummies, were in fact live nuclear weapons.
The 5th Bomb Wing subsequently failed its NSI in May because of failures to properly secure its nuclear stockpile.
The 91st Space Wing at Minot failed its limited nuclear surety inspection in January, also because of “critical safety and security” failures.
When inspection teams returned to Minot in May, they determined the wing had fixed its security flaws and passed the 91st. Jesus is coming soon. Are you ready?

WASHINGTON — Three ballistic missile crew members fell asleep while holding classified missile launch codes two weeks ago in North Dakota, the Air Force said Thursday.
The error triggered a military investigation, which found the codes were not compromised.
But the July 12 incident comes on the heels of a series of missteps by the Air Force that had already put the service under intense scrutiny.
In this case, officials said the crew members reported themselves for the violation of procedure. The crew members were apparently leaving the control center at the end of their shift and took a set of old codes, which are typically replaced after every shift, to a rest area in the facility. Rather than proceeding back to Minot Air Force Base to dispose of the codes they fell asleep.
Air Force officials said the deactivated codes were not at risk of being lost or stolen and that they were always contained at a secure facility.
But the Air Force did not initially make the incident public, and on Thursday the Project on Government Oversight issued a report saying the Air Force base was on "security lockdown" after discovering a "nuclear weapon launch code" missing.

Missile crew falls asleep with nuclear code device: air force
WASHINGTON (AFP) — Members of a US Air Force nuclear missile crew face disciplinary action for going to sleep while in possession of an invalidated nuclear launch code component, the air force disclosed Thursday.
The breach occurred July 12 at Minot Air Force Base in North Dakota, the scene of two other recent high profile lapses involving nuclear weapons or nuclear-related components, according to the spokesman.
An investigation into the violation of procedures "concluded that no compromise of the classified material occurred," the air force said in a statement.
The nuclear launch code component is a device that enables the missile crew to issue an electronic command to launch a nuclear missile. The codes are changed on a scheduled, recurring basis.
The four member missile crew changed the codes in the underground launch control center during their watch and had gone "top side" to an above ground missile alert facility with the component containing the old codes to await transportation back to the main base, the air force said.
While awaiting transportation, they went into a crew rest area with bunks where they fell asleep, a violation of the rules, according to the air force.
"An investigation revealed the codes had remained secured in containers using locks which combos were known only to the crew during the entirety of the incident," the air force said.
"Additionally, access to the MAF (missile alert facility) was continually controlled by air force security forces and the codes had been superseded and were unusable," it said.
The breach was reported by one of the missile crew members, Paoli said.
"We would say it's a procedural issue, but when you're dealing with nukes all these different procedures are steps in the safeguarding process, and they are important," he said.
Paoli said the crew members "undoubtedly are facing administrative disciplinary action."
The air force's handling of US nuclear weapons has come under intense scrutiny because of a series of mishaps.
US Defense Secretary Robert Gates took the unprecedented step last month of firing both the air force chief of staff and the service's civilian secretary for failing to get a grip on a 10-year erosion of standards in the nuclear forces.
Last year, nuclear-armed cruise missiles were inadvertently loaded onto a B-52 bomber at Minot and flown to Barksdale Air Force Base in Louisiana.
In March, the air force discovered it had mistakenly shipped nose cone assemblies with fuses to trigger nuclear weapons to Taiwan in 2006 Jesus is coming soon. Are you ready?

If the discrepancy in the number of nukes flown was real and there is an unclaimed nuke out there then how does the code go bad? In other words how can the code go "old" if it isn't updated in the weapon?
I am asking for assistance understanding how these kinds of things work to assess the reality of this being any concern.
