People are concerned about this situation and we are worried as well. Our concern has grown since yesterday as much has been learned since then.

There is much uncertainty.

We are moving quickly to learn as much as possible.

This will be a marathon not a sprint.

Recommendations will change as we learn more about the virus and its transmission.

Local adaptations are important. May be confusing or conflicting information available.

US Gov is working with the WHO and other international partners are working to learn and respond to this international threat.

We do not know if this swine flu virus or some other influenza virus will lead to the next pandemic.

Yesterday reported 7 confirmed cases.

Another additional case confirmed in CA, bringing total to 8.

Do not have enough information to fully assess the health threat.

Tested 14 samples from Mexico; all confirmed positive for swine flu.

We do not have enough information to give us a sense of the extent of this virus.

CDC has sent teams to CA and TX to assist state and local health officials to identify those who may be infected. May send a team to Mexico.

Outbreak notice will be issued to US travelers to Central America and Mexico City.

CDC is not recommending any additional cautions to CA, TX or Mexico.

Vomiting and diarrhea is also associated with the swine flu.

Why more concerned?

A what we learned is that the cluster of cases we are seeing in Mexico is swine flu virus. What we are seeing in the US is a mild swine flu. It is critically important that we find out details of what’s going on in Mexico because it is more severe there.

Q Samples from Mexico – any preliminary sequencing been done – how closely related to CA cases? Also talking to vaccine producers about ceasing production for next fall vaccines and working on a swine flu vaccine?

A We are seeing similarity between the viruses. Have created a seed stock of the virus as is done with new influenza viruses. At this point have not made any decisions regarding vaccinations but thought that it was prudent to made the seed stock.

What pandemic threat level are we at?

No change in the pandemic threat level. WHO will be looking at the situation occurring in Mexico and will be looking at convening a meeting.

Q give us some sense of the number of cases and deaths? Conflicting reports.

A We look forward to understanding more about the numbers from Mexico. I refer you to the WHO and Mexican government for that. We are not in a position to offer a number of cases and deaths in Mexico.

Q has been a report that the attack rate and severity is greater in young adult and teenager. Is that true?

A Understanding who is getting sick in Mexico is premature at the moment. But what you usually see in a pandemic is that age group getting sick as opposed to a seasonal influenza.

Q Is there a common thread in the cases in CA and TX?

A That’s what we’re looking for we have not been able to identify that. We have not identified a common exposure. That’s what we need to know…. Looking for undiagnosed cases….

Q Elaborate on analysis on strain from Mexico? All US cases match?

A Yes. All seven isolates matched the genetic component from Mexico that we analyzed.

Q why a difference in severity between US and Mexico?

A That’s one of the difference that the labs are looking for.

We have not received details about whether our samples were from patients who have died but we know they are seriously ill.

My understanding is that one of the cases from San Diego had traveled to Mexico.

Q we hope this is going to be limited in Mexico and nothing will happen but what would trigger a higher level of concern? What would that look like?

A There has been extensive planning across the federal government and private sector… planning for influenza pandemic for years and we don’t know if this will become pandemic but the level of planning is unprecedented and will allow us to respond in ways we couldn’t have ten years ago…. I refer you to our website on pandemic preparedness for specifics on preparation.

WHO is not on the point of declaring a pandemic. We are on the point of discovering more about this virus and how to control it.

Q It seems as though from what has been reported that it is more likely than not that the condition has been meet to raise pandemic alert to level 4. When do you think that you and the WHO will have enough information to make a decision?

A We have been in close contact with the WHO and in close contact with the director regarding this. They convened an expert panel to look at the conditions and determine whether or nto we should raise the status. You are seeing a lot of activity around San Diego, in Texas and in Mexico because that is where it is occurring. What we are seeing is individual preparedness…. What can we as individuals do to prepare for if there is a pandemic. This is a time for people to be thinking about preparing right now.

We are moving quickly and being aggressive in our approach and taking this very seriously. I expect that each day we will be evaluating the situation and making decisions.

CDC has personnel all over the globe and our interactions with Mexico are strong.

Q How concerned should people be outside of Mexico, CA and TX.

A They should pay attention. We are worried about this and treating it very seriously and people need to pay attention to what’s going on.

Q Strong interactions with Mexico but has Mexico asked for CDC helped and has anyone talked about containing this virus?

A Not in Mexico yet but anticipate will be there very soon. There are things that suggest that containment is not very likely because we are seeing in CA and TX without any common links between them. Transmission is not likely to be contained.

Q When you talk about spreading person to person through several cycles…?

A When you can find no connections between the sick persons.

Q Why is Canada evaluating Mexico’s viruses.

A They provide support to other countries just as we do.

Q 14 samples from Mexico, 7 positive. Can we say in a simple statement that it is the same virus in Mexico, CA and TX

A I think we can safely say that. As information changes we will share it but so far genetic elements are the same.

Q What were the other 7 samples?

A Other 7 samples came up negative for swine flu but we can’t really say anything else about them.

Q Details on the newest case in San Diego

A refer you to the health dept there I don’t have those specific details but I believe the child has recovered and was not hospitalized. We are looking at patients in the areas where we’ve seen cases so far but it’s important that clinicians have a high index of suspicion. We are just learning about this virus and transmission.

Q Cases in Mexico – any of those people had contact with swine and much travel between Dallas and Mexico – anything local officials should be doing near buses, airports, areas of travel?

A No details about cases in Mexico. In terms of travel screening, there are 400 million crossings of the border between US and Mexico a year. Best approach is to encourage people who are sick not to travel.
