Oh and they ant SS even on your contacts or your next of kin...it is bizarre-I understand wanting yours or spouses...but contacts??? Seems very odd...




Handing out health forms for decades and many of these questions are not standard-then the casual psych eval seems over the top for a primary care provider unless you have DPOA or something-it seems more and more folks are under DPOA and it is standard question at the ER now...weird...foreshadows poorly for the people imho
