"First it was hockey players. Now, wealthy hospital donors have jumped the queue to get H1N1 flu shots.

On Friday, news emerged that the top 200 donors at Montreal's Jewish General Hospital have already been inoculated.

Meanwhile, board members of Toronto's Mount Sinai Hospital were offered H1N1 flu shots at a regularly scheduled board of governors meeting Oct. 26, says the union representing 46,000 health care workers in Ontario.

"The message this is sending is that the wealthy and influential should be treated first," said Pat Chastang, a spokeswoman for the Service Employees International Union Local 1 Canada, "even if it means pregnant women and children must be made to wait."

The Ontario union urged all 65 of Mount Sinai's board members, who received their vaccinations on Oct. 26, to resign. "The most important quality of a board member are their ethics and their integrity. Clearly, jumping the queue was a mistake," said Chastang.

A statement from the hospital said the vaccinations were given during a clinic that was open to the general public during a time where no shortages had been announced.

"The focus was on inoculating health-care workers, patients, and any member of the public who requested the vaccine. No one would be turned away; it was understood there was sufficient vaccine available," said the statement on Mount Sinai's website.

In Montreal, a spokeswoman for the Jewish General Hospital said all 200 donors who received H1N1 shots serve as volunteers in some capacity.

"It has nothing to do with the fact that they gave money," said Karen Ohayon. "There's absolutely no link."

However, Marie-Eve Bedard, press attache to Quebec Health Minister Yves Bolduc, said board members of hospitals and donors are not entitled to vaccines ahead of the rest of the population.

"There has been a gap in communication because this morning we learned that certain (hospital) boards of directors were vaccinated, which the minister considers unacceptable," Bedard said.

The revelations come as the World Health Organization warned "intense and persistent" flu transmission continues to be reported in North America, "without evidence of a peak in activity."

This week, Canada's death toll reached more than 100 since the beginning of the pandemic in late April. As of Nov. 5, 115 deaths due to H1N1 had been reported to the federal government. The number of new hospitalizations were more than three times higher than the previous week, and 737 new flu outbreaks were reported — 710 of them in schools.

Virtually all flu cases diagnosed in Canada — 99.7 per cent — are H1N1.

The Public Health Agency of Canada had received reports by Oct. 31 of 2,440 hospitalized cases, including 443 admitted to intensive care and 230 that required mechanical ventilation.

In its weekly situation update released Friday, WHO said rates of influenza-like illness, the proportion of respiratory samples testing positive for flu, and outbreaks in schools "continues to increase sharply in Canada as activity spreads eastward."

Across Canada and the U.S., flu rates are higher than normal for this time of year, with 48 states reporting widespread influenza activity. Significantly more cases of H1N1 have been recorded in Mexico since September than were seen during the initial outbreak of H1N1 in the spring.

"Everything we are seeing is the 2009 H1N1 virus," said Dr. Anne Schuchat, director of the U.S. National Center for Immunization and Respiratory Diseases at the Centers for Disease Control and Prevention, during a teleconference with reporters Friday.

"We wouldn't have expected this many states to have this widespread of a disease," she said. "Flu can last until May. We don't know what we will see with this virus in general."

As H1N1 activity increases, tempers are also rising over who is getting access to flu shots ahead of targeted priority groups. Alberta's health authority has fired a second person for involvement in giving Calgary Flames players and their families the H1N1 vaccine last week.

A statement released Friday by Alberta Health Services detailed how the hockey players got their shots without having to stand in hours-long lineups with other members of the public.

"A Calgary Flames medical representative approached Alberta Health Services on Oct. 27 (the second day the vaccination was available to everyone in Alberta) through a staff member to see if an H1N1 immunization clinic would be made available to the Flames and their families," the statement reads.

"The request was forwarded to a more senior AHS staff member, who in turn took it to a supervisor and the request was approved."

The clinic for the players and their families was held on Oct. 30, one day before the province closed the doors of all mass immunization clinics because of huge public demand and a vaccination shortage.

"There is no written record of approval to proceed and there is no process for authorization of the use of the H1N1 vaccine for this purpose, as it contradicts existing protocols and processes," the statement said. "Use of the vaccine was not discussed or authorized above the level of the individual who authorized its use."

The name of the most senior staff member involved, who was dismissed earlier this week, has not been released.

The investigation is complete and no further disciplinary action is being considered.

Currently, only pregnant women and children aged six months to five years can get their shots at Alberta clinics, which were reopened Thursday.

Meanwhile, hospitals, schools and businesses are grappling with absenteeism. And workers with the Alberta government have been told that they will need to swear a legal oath that they had the swine flu if they miss more than three days of work.

The policy is meant to ease the pressure on family doctors by removing the need for medical notes during the outbreak, said government spokesman Gerald Kastendieck."
