He worked as epidemiologist in an ebola treatment center in Sierra Leone. That's were he got infected, very likely by an collegue. Ten days later he got sick, another ten days later he came to Hamburg.
"The patient's fluid balance was extremely disturbed, his digestive system affected", Benno Kreuels, doctor of UKE and author, states. Fluids and electrolytes could only be given via infusion: During the first day, 10 liter or more were required. Because of immensive diarrhea the patient lost from 4 to 8 1/2 liter fluids per day."
Severe complications
The typical ebola symptoms like diarrhea and vomiting already weared off after a few days, the viral load of the patient decreased. But due to a sepsis the condition of the patient worsened at the 13. day. The bacteria were resistant against different antibiotics.
The patient suffered from impaired conciousness and limited lung function. "Only the intensive care saved our patient", doctor for tropical diseases Stefan Schmiedel says.
The patient was given artificial respiration among other things.
The doctors assume, sepsis contributes to the many deaths in West Africa. Because of the ebola infection the intestinal mucosa is extremely inflamed, increasing the risk for sepsis.
Hamburg's patient was released 40 days after onset of symptoms - for at least 20 days no ebola virus could be detected in blood, saliva, sweat, stool, urine, tear fluid or conjunctiva of the patient. The medics learned a lot about the virus and the course of disease, says Marylyn Addo, head of the section for tropical diseases at the UKE.
UKE doctor soon helping in affected areas
The expenses for the treatment could sum up to about 2 million Euro, according to SPIEGEL information. The hospital management first estimated the costs for the treatment to about 300.000 Euro, but there have been unexpected expenses. The WHO promised to pay for the treatment expenses.
Doctor for tropical diseases Schmiedel soon will help out in West Africa. Wednesday he is going to travel for the organisation "Doctors without Borders" to Sierra Leone and will stay for three weeks, he stated thursday - to help out in a hospital, which is located at the center of the outbreak.
Treatment possibilities there are extremely limited due to lack of personnel. He doesn't worry to get infected by the dangerous ebola virus. "I don't have any fear, because I know how to protect myself from it".
