Apart from the un indentified modification, the only other thing remotely suspicious is this statement on page 48:

"Incident occurred during the suspension phase of the well - 2 hrs after completing an integrity test on the well"

Why was there a suspension? did special ops use this suspension time to move in and "do their thing/sabotage"?
