In those days the wealthy got treatment, the poor didn't. Simple as that.

Whether the treatment worked or not is a different question.

Now pretty much anyone can get treatment (if they can get to a metropolitan area) and the treatments are better. I don't mind chipping in a few bucks to help pay for some indigent old bag's treatment. I may need help myself someday.
