They can crate a nuke up and get it on board a ship pretty easily. It's not hard to sneak something the size of a fridge on a ship that has absolutely no connection to NK other than the fat paycheck they're getting to do it. Nk has shipping ports and fishing ports... Get a crate on a fishing ship, have it randevu with another larger ocean crossing ship, and whala, you got a nuke cruising through San Fransisco bay right into downtown SF.
