Pain control is very different if you are a poor dying person and a rich dying person. There is no reason for a dying person to be denied whatever dose of painkiller they need whenever they feel they need it - they're dying anyway, let them be in comfort if it's what they choose. But there are different types of pain, and some can't be touched by drugs.
