2-copies of medical records
3-lawyer
4- sign consent forms so the lawyer can obtain copies of medical records-this should be the only point at which the hospital has the heads up you are pursuing this. Until your lawyer has papers in hand do not speak of it to anyone else within that hospital.
5- give the lawyer your copies for comparison on any changes or edits to the original paper work
