Is respiratory protection appropriate in the Ebola response?

We write to express our concern about one aspect of the response to the current epidemic of Ebola that has, so far, received little attention,1 lacks an evidence base, and might be counterproductive.

The primary mode of transmission of Ebola virus is through contact with infected patients’ secretions (such as blood, vomit, or faeces) directly and indirectly (for example, from contaminated needles). This transmission occurs via close family contact or in health-care settings, particularly when placing orotracheal intubation or when caring for a patient who is vomiting or bleeding. Ebola is rarely transmitted via an airborne route.2 Although these routes of transmission are well known,3, 4 most agencies, including governmental agencies responsible for repatriating western patients, apply infection-control measures appropriate for airborne diseases.

Excessive precautions could offer reassurance to those responding to Ebola, yet complete respiratory protection
is expensive, uncomfortable, and unaffordable for countries that are the most affected. Worse, such an approach suggests that the only defence is individual protective equipment, which is inaccessible to the general population. Moreover, the image of workers with spectacular protective clothing might contribute to the panic in some communities. If this leads people to flee affected areas it could increase the spread of infection. It also reinforces the view that some lives are more valuable than others, already engendered by decisions about the use of experimental Ebola drug ZMapp.5


We contend that the systematic application of precautionary measures that protect health-care personnel and others from direct contact (ie, gloves and waterproof smocks, goggles, masks, and individual rooms or wards in the hospital) are sufficient to manage most patients (who do not experience haemorrhage or vomiting). In fact, goggles and masks might not even be necessary to speak with conscious patients, as long as a distance of 1—2 metres is maintained (the maximum distance that infectious droplets might reach). Exceptional precautions, such as pressurised suits with oxygen tanks, should be reserved for interventions that generate aerosols (invasive explorations or intubations), specific situations (eg, massive haemorrhage), or in laboratories where the virus is cultivated. They are unnecessary in the settings where the virus is most rampant.

In western Africa now there is a need for rational and efficient use of protective equipment. This can only be achieved by communicating a consistent message that the disease is essentially transmitted through direct contact.

Comment: There is already panic. If it takes full BSL4 moon suits to keep doctors safe, then spend all that money on those suits. People are already fleeing. Telling people that masks might not be necessary for conscious patients as long as you stay away 1-2 meters is crazy, and irresponsible. This article is a joke. If simple Precautions are all one needs, let's get the authors off to the Hot Three to walk the walk.

The first 3 reasons(excuses) are why IMF, CDC, and the gov'ts of these countries have been asking for help. These countries need the suits, but can't afford them. So saying that since they can't afford them is a good reason for them not to wear them is circular reasoning. Illogical.

The next excuse sounds like our "everybody gets a trophy or nobody does society."

We don't want people to know that they need a protective suit is more truthful, and a piss poor reason to not provide them to the HCWs.

And the last excuse is it won't look good to the community.

This is nonsense.

First the population might feel better about going to a hospital that has the proper equipment and of course the patient's without Ebola would be safer.

Second, who cares about the suit's curb-appeal when you're asking people to care for Ebola patients.

These are 3 excuses that libtards use that makes me think they are mentally ill because they can't live in reality.

Sept. 4, 2014 -- A plane carrying Rick Sacra, MD, the third American health worker infected with Ebola, left Liberia on Thursday, his wife said at a news conference.

“I just had a call from the doctor who put Rick on a plane to come to the United States. And he said Rick is clearly sick but that he was in good spirits, and he walked onto the plane. So we are really encouraged by that,” Deborah Sacra told reporters, her voice breaking briefly as she held back tears.

Sacra told his wife he thought he might have been infected by a patient with HIV who had also contracted Ebola, according to Doug Sacra.

One of the ways ELWA hospital tests patients for Ebola is by checking their temperature before they’re admitted. If they’re not running a fever, they’re treated as being free of the disease, he explained.

A fever is one of the first symptoms of Ebola infection. It’s the body’s way of beginning to mount a defense against the virus.

A patient with HIV, who has a weakened immune system, may not have been able to run a fever in response to the infection, and thus would have mistakenly been thought to be Ebola-free.

I can't f-ing believe this. Ever go through customs on returning to the US? They will give you the third degree if you have too many packs of cigarettes, or refuse them to copy your computer's hard drive. And this joker, a walking weapon of mass destruction, slides through into the US like crap through a goose.
