The CCTV shows that the kid, believed to be 1-year-old, is at first attempting to climb the shorter wall of the pool. After successfully climbing it, the kid is seen walking carefully on the pool’s edge. He even carefully crawled on the top edge of the pool just to make sure he might not fall. My son was 2 years old at the time. One day, I was returning a video to the video store, and my now ex-husband was watching our son. When I got home, my son wasn’t in the house, and my then husband said he was outside playing. I started to panic immediately, went looking for him, and found him face down, blue, lifeless, and floating in the neighbor’s pool. I jumped in the pool and began doing CPR, but got no response. My neighbor called 911 while I continued CPR. As I continued rescue breathing, I saw that Sean’s stomach was distended. In my panic, I had been giving adult breaths rather than child breaths. Listening to a voice, which I know was God, I stopped everything and did the Heimlich maneuver. As I did, he regurgitated a large amount of pool water, and almost instantly, his pulse returned. He regained consciousness about an hour later in the ER, and was discharged in the morning. Many of you know him, and he is a happy, healthy young man today. My friend did not have the same experience. The year prior her 2 year old daughter drowned in a similar manner. Despite being a nurse, and performing CPR as I did, her child died. I was enrolled in my first research class, so I determined to find out why some children survive, and some do not after drowning. I undertook a year-long analysis of every child drowning in the state of South Carolina. What I found was stunning. In the cases where the Heimlich maneuver was performed prior to rescue breathing, the child survived. If the Heimlich maneuver was not performed, the outcome was dismal. I thought this was significant, so I wrote to Dr. Heimlich, and he wrote me back. He had been trying to change the American Heart Association protocol for treating drowning victims to no avail. They flatly refused to change their protocol, and he was very frustrated. Dr. Heimlich is now 94 years old, and the controversy continues. I’m unsure why transferring a child to the morgue is preferable to performing the Heimlich maneuver, but Dr. Heimlich has been challenged on many fronts regarding this issue. How and why does this work? Going back to scientific principles, the answer is the ventilation/perfusion ratio. First, in about 90% of child drownings, no water goes into the lungs. Considered “dry drownings,” children have a natural reflex whereby the epiglottis closes, blocking off the lungs to water. Of course, the lungs aren’t getting any oxygen either. Where does that water go? The answer is, into the child’s stomach. The stomach can become so distended with water, it presses on the lungs. Normally, we breathe in air, and the alveoli (little air pockets in the base of the lungs) drop off that oxygen to blood passing by in the blood vessels. This is called the ventilation (air), perfusion (blood) ratio. When the child is rescued, the epiglottis relaxes, so we can do rescue breathing, but if the blood vessels are blocked by a fluid-filled stomach, all the oxygen in the world won’t get into the blood stream where it is needed. The Heimlich Maneuver rapidly empties the stomach of a drowning victim, and should be done in the face-down or side-lying position. If there is suspicion of a neck or back injury, as in a diving accident, the spine must be protected to prevent paralysis. How long does the Heimlich Maneuver take? Approximately 4-6 seconds. Odds are, you may be in a position to aid a drowning child. Most drownings occur between the ages of 2 – 4 years old, and water and unattended children do not mix. It is hugely important to know that a child who has drowned should not be considered dead until AFTER CPR has been performed and the Heimlich maneuver has been used. Let’s have a safe summer!
