Expert observers, state officials and civil society activists who are in contact with citizens in the self-proclaimed Luhansk and Donetsk People's Republics describe an atmosphere of "chaos" in those regions, with militant leaders who take marching orders from the Kremlin struggling to maintain order.

"Despite all attempts to create police, they could not do it," Dmytro Snegiryov, head of the Prava Sprava civil initiative, an NGO that supports the army and monitors conditions in the conflict zone, told SETimes. "Today the main acting principle there is that a person with a weapon can dictate rules to civil citizens. Most mercenaries in the LPR or DPR are from Russia, they came there to rob. Laws for them are empty words. They received arms, and can do what they want."

"They have the martial law there, so if somebody from the rebels takes your property, from their point of view it is legal," Sergiy Garmash, editor-in-chief of the Donetsk news agency Ostrov, told SETimes. "We have the facts when rebels take apartments, or militants could settle in empty apartments. They decided that if, for example, people have left their homes and communications in the area are destroyed, they may take such property."

"When the ceasefire was active, volunteers and community organisations delivered humanitarian aid to those areas at least," Berg said. "But now it is difficult to predict how the situation will develop in the future."

The biggest problem that we have seen and what I myself have seen, having visited hospitals and institutions, is the lack of medicine. If you visit the pharmacy of a hospital with 300 or 400 beds, there are just shelves with a few boxes. Doctors are expected to treat patients with hardly anything. It’s the same at the primary healthcare level - not just in hospitals.

Surgeons and doctors can’t work. There’s a huge shortage of anaesthesia. One doctor told me they needed to operate on a child but they couldn’t because they didn’t have any anaesthesia.

In some hospitals 20 percent of the staff left; in others up to 50 percent.

"We [were] shelled and I'm [thinking] 'Oh my God, this is too close,'" said Artur, a rebel lying in a bed in Krasnyi Luch City Hospital, some 22 miles from the town of Debaltseve. "I only remember that roaring sound of incoming mortar [fire], explosion, dust and suddenly it became very dark."

Carnage surrounded the fighter when he regained consciousness in the forest outside town.

"I'm looking around — pieces of meat everywhere, just pieces of meat, some bodies ... [were] just laying there," he said, requesting that only his first name be used.

Since Monday, more than 70 fighters have been treated at this hospital alone — a major increase, according to staff. Staff say they don't know how many fighters have been killed.

In nearby Krasnyi Luch City Hospital, two rebels who did not want to be named said they had witnessed an increased number of casualties. The recent escalation in violence comes as separatists intensify their offensive against Ukrainian forces near rebel strongholds of Donetsk and Luhansk.

These setbacks are creating divisions among rebel troops, the rebels say.

"I'm a little bit demoralized because so many guys suffered, a lot of guys I used to know [were] killed," Artur said from his hospital bed.

A rebel fighter in another room also described intensifying fighting during a battle over a rail hub about 18 miles from Debaltseve.

"On the way we faced intensive resistance [from] the Ukrainian army, we had to fight back ... others were also wounded but I don't know where they are," said the separatist, who did not want to reveal his identity for fear of retaliation.

He was shot twice in the leg and shrapnel is embedded in his back. But he was lucky — many other fighters did not survive, the rebel said.

"Losses were big," he added.
