On October 5, 1966, Fermi 1, a prototype fast breeder reactor, suffered a partial fuel meltdown, although no radioactive material was released. After repairs it was shut down by 1972.[1]

Multiple problems at site, Monroe MI/Lake Erie (SE Michigan)

Reactor is in Hot Shutdown

Large leak in turbine pump hose, but there's more.

Unisolable leak in feedwater system. Meaning, they can't find it.

Water level was 137 inches at lowest, at least that is what is being reported...meaning fuel have been uncovered.

Anyone local with eyes on this plant?

The NRC event is detailed here with add'l info detailing new leaks, etc.

If the cooling system is compromised, well. More on that later, hopefully it won't be necessary to discuss the ramifications of having a nuclear meltdown next to the largest freshwater lake system in the world. Oh, wait, they already had one there in 1966...

Nuclear Engineer Dave Lochbaum with Union of Concerned Scientist interpreted the initial Event Report as:

Somewhat unusual to lose the Turbine Building Component Cooling Water (TBCCW) System.

Attached are sections from the Fermi 2 UFSAR on the system.

It has three pumps and two heat exchangers. Normally, two pumps and one heat exchanger are in service.

There's no interlocks that would allow a spurious or unintended electrical signal to trip the pumps or close isolation valves.

It would appear that the system was lost when the pumps stopped running. The piping and instrumentation diagrams indicate the pumps trip at less than 7 psig suction pressure. But the system has a head tank designed to ensure that the pumps have adequate suction pressure. So, absent a leak that drained water from the system, the pumps should not have tripped due to low suction pressure. The pumps have electric motors that receive 460 volt power. The initial report did not indicate that there was any offsite power disturbance. Perhaps there was an onsite electrical distribution system problem. I didn't pull the electrical drawings, but it's common to load two of three TBCCW pumps on a single supply bus. If so, the failure of that single bus could take both running TBCCW pumps out. Even if the third TBCCW pump in standby could be started, it alone could not handle the loads at full power.

The domino problems caused by the TBCCW loss (e.g., need to turn feedwater pumps off, secondary containment isolation dampers closing, need to start Standby Gas Treatment) are all expected actions given the loss of this non-safety-related system. (Good thing they didn't lose or misplace a safety-related cooling system.)
