Just express your wish to go there to be healed and next time you fall asleep you will go there. There are no limitations other than the ones already stated.
Real vanity wishes will not be worked on. Wishes that make you look more normal will be worked on. It does not have to be a disease or a serious ailment per se..
I hope you get better and the angels or other worldly entities of that facility help you out.
