the biggest issue i can see is that they haven't given this this thing enough time to Cure, before running it at full throttle.

Also it was over 100 degrees for most of the summer when they were working on the spillways, don't know if that makes any difference. But Oroville Dam is very remote, they had to build a concrete plant and make the concrete right there, it wasn't trucked in.
