I was an aerospace engineer who worked with several Apollo astronauts as well as many of the workers on the program.

The Apollo fire was due to NASA's incompetence and arrogance.

The engineers at North American Rockwell had sent several letters to NASA explaining the extreme fire danger of 17 PSI oxygen. NASA ignored them. There was test data showing that velcro burned like gasoline at that pressure of O2.

The astronauts in the test capsule had used velcro liberally on the inside of it to hold pens, charts, and other gear.

In addition, NASA had designed the capsule door to only open inward. They were afraid an astronaut would inadvertently open it in space, so the air pressure inside would keep it closed as long as the capsule was pressurized.

My friend, Ken S., was one of the first techs to enter the burned capsule. He told me that it looked like someone had kicked an avionics box off the wall. This could have happened when the men were being burned to death or it could be that an angry astronaut kicked it and that started the fire. No one will ever know.

After the fire, North American Rockwell fired a VP who had no responsibility for the fire and fell on their swords. That took the pressure off NASA and insured that Rockwell would be chosen as prime contractor on the Space Shuttle Program. Of course, that was not the only reason, but Rockwell knew how to keep NASA happy.

At any rate, there are plenty of books written about the tragic accident. Try reading a few.

Grissom, Chaffee and white were killed in Apollo one in a capsule fire in an oxygen environment. It caused a complete redesign of the Apollo capsule and procedures.
