For the majority of morality is based on feelings. If I slap you in your face your feelings will be hurt, you will not like, therefore don't hurt people because it's objectively wrong.

However, sometimes we need to hurt people in order to help them, so then the intention is a factor. A doctor setting a bone or doing surgery without anesthetic, spanking a child out of love to punish their bad behavior that they may fear pain to avoid doing stupid things.
