I think we're dealing with displaced hemoglobin. More like attitude sickness or carbon Monoxide poisoning. He is referring to the fact that the end positive pressure of the vents causes trauma to the lungs. Which is a cost we pay for the benefits of getting the out of ARDS... ARDS is negative space between the aveoli and capillaries in the lungs, no exchange at that level. What he is saying is it not that or a VQ problem and the added pressure on the lungs isn't necessarily. O2 support is necessary but maybe because of the red blood cells inability to transport 02.
