Any nurse or doctor knows this is bullshit story.
An order for medicine is put in computer including dosage as well as concentration of the medicine example like mg/ml and how many mg and ml to administer.
Ok so pharmacy distributes it in vial with bar code. The computer with the order is connected to the scanning wand. The vial is scanned prior to administering to each patient and will alert and not go forward if barcode does not match. Nurse verifies with her eyes tbe name and dosage on the vial and signs. Many medications require 2 nurse signatures fir safety. Im sure this one!
The computer requires lot numbers and all identifying information and expiration dates off-of the vial.
Quoting: Anonymous Coward 13585916 Every state and county health authority has different rules and regulations as do hospitals and doctors offices. In many hospitals the regulations are set entirely by the administrators who only have to follow certain laws around certain heavily controlled medications and everything else is manually retrieved by a nurse or a doctor. There are wildly varying standards even with pharmacies. Mix-ups with medications, including injectables are quite common. A woman I knew for nearly thirty years died when a nurse mixed up an injectable medication and gave her the wrong shot.
