It might be chronic. With massive clotting, the clotting factors in the blood can be consumed. Also, once blood thinners are started, there is a genuine risk of bleeding. And if the hemoglobin is low say like 5-6, the person has no reserve. If they start to bleed at home (due to fall, head accident or ulcer), they may not have enough reserve to make it to the hospital. So a few units of blood in this case is an insurance policy so to speak. It is not generally given for clots. The story is much more complicated that simply that.
