The president is the final arbiter of what is and what is not classified, the level of classification as well as who has clearance to see what. The president does not "have to" go through a formal process for any of this. If the president hands you a "classified" document, that means you are cleared to see it. If the president leaves "classified" documents laying around for everyone to see, that means those documents are no longer classified.

I made classified todo lists for my men on classified papers. It’s totally legal.

Just because documents are marked classified (classified is not a marking) doesn’t mean they are classified. A doc can be marked as secret which means every page in the document is marked top and bottom as secret, even the blank pages and the backs of pages. The paragraphs are marked (S) or (U) so that you know what is actually classified as secret.

Probably 80% of all classified documents are unclassified but have classification markings.

My last boss would make TS/SCI (etc) notes all over documents. It’s totally normal and work would cease if you couldn’t mark up documents. A lot is done on the computer now but my boss liked print-outs.
