I´ve cleaned the mouse mat, ball under the mouse and defragged the computer, but it is no better. Does this mean I should buy a new mouse, or what? Thanks in advance for any advice.

You should be able to twist it off. The rollers are probably coated with crud. Scrape it off. next time you buy a mouse, buy optical.
