If its an online survey, send the link to your states EEOC and ask. Pre-employment health screens, drug screens, mental evaulations or personality evaluations are illegal in most states.

Note, I said PRE-employment. Once they make an offer they can request these items as a condition of actually getting the job, but they must make a job offer first.
