Early results of a test of a portable triage centre meant for outbreaks of infectious disease have been successful, health officials in Edmonton say.

The regional health authority is testing a portable triage centre as part of the city's flu pandemic preparations.

The centre includes a series of tents set up outside a hospital, where people with a cold or flu are assessed and treated without risking bringing their germs into a regular health centre.

"We wanted to develop and test a method of dealing with higher numbers of people who need to be assessed for fever and cough related to a potential influenza pandemic," said Dr. Marcia Johnson of Edmonton's Capital Health.

In the three days that the clinic has been open to the public, it has performed admirably, said Shawn Grono, the project's director.

"I've worked in emerg for a long time," said Grono. "It's just like being in emerg where you have patients, you have carts full of supplies, you have technical equipment, you have vital sign monitors."

The clinic can handle up to 600 patients a day, and it is equipped with a lab, X-ray technology and a link to the digital imaging station.

The system packs into a couple of trailers and can be set up in six hours, complete with its own power, water, heat and air circulation system designed to prevent the spread of germs.

The tent has been warm, comfortable and busy despite -18 C weather outside.

"I think it's a great thing because it gives people somewhere to go to get good service and to get to the bottom of their problems," said patient Bill Penrice.

The tent will stay open for two more weeks, and then will be tested again in the summer under extreme heat conditions.

EDMONTON, Alberta, March 4 (UPI) -- Stollery Children's Hospital in Edmonton, Alberta, overwhelmed by a flu outbreak, is using outdoor heated tents to process new patients, local media said.

City health authority spokesman Steve Buick said the past two weeks have been extremely busy because of seasonal illnesses. He said because the children's facility shares a waiting room with the University of Alberta Hospital, the tents were a good way to alleviate crowding, the Edmonton Journal reported.

"Children cry and suffer stress in a way they can't control and it's disturbing for the adults waiting in the waiting room," he said. "It would be better for the kids to have their own space."

Buick said the tents were purchased last year as part of a emergency planning for pandemics and are only a temporary solution, the report said. He said plans are being drafted to expand the Stollery emergency department and to build a separate entrance and waiting area

It's early 2003. Chinese health authorities announce that a pneumonia-like illness has sickened 305 people and killed five in that country's southern province of Guangdong. Panic sets in and uncontrolled rumours cause residents to hoard food and supplies while their government says everything is under control. A few days later, the first case of severe pneumonia is reported in Hong Kong. Vietnam is next. And within a few weeks, the World Health Organization issues a global health alert after an atypical pneumonia does not respond to standard treatment and spreads to medical staff.

It isn't long until the numbers rise by the hundreds, schools are closed, apartment complexes are quarantined and the rest of the world hears almost hourly media reports about the deadly disease sweeping the globe. SARS, or Severe Acute Respiratory Syndrome, becomes a feared acronym and Toronto starts feeling the effects when a travel advisory is issued for that city. Health Canada maintains quarantine services at most international airports to be able to respond quickly to a severe SARS outbreak. When the crisis reaches its end, the eventual toll is 8,098 cases and 774 deaths.

SARS was one of the biggest health issues facing the global community. Because medical staff were not immune to the highly contagious and mysterious disease, health-care professionals were unsure of the most effective ways to prevent its transmission in the early days of the outbreak.

Closer to home, Capital Health wants to be better equipped if an infectious disease outbreak or pandemic strikes the region. That's where triage tents, or to be more specific, the Portable Isolation Containment System (PICS) fit in. Capital Health has ordered a system that will allow the region to increase its capacity and help isolate sick and potentially infectious people seeking help at emergency rooms from the already-admitted patients and staff treating them. Serving as an entry point for the throngs of people that might flock to the hospital for treatment, the three separate tents will provide space to assess the people and then move them – either home, to short-term care or the hospital.

"The biggest thing these triage centres would do is protect our infrastructure for as long as possible during an infectious disease outbreak," says Eric Bone, regional director of the office of emergency preparedness for Capital Health. "This system would allow us to help keep infectious disease out of the hospital and protect other patients from becoming infected. There has been a lot of talk about planning for an emergency but we are unique in that we are taking it one step further with these triage tents."

Local police knew their training would be tested Saturday. But with the acting and adrenaline, the drill almost seemed like the real thing.

As part of a joint school-police-hospital disaster drill, Unity experienced a mock terrorist attack, bomb threat and a hostage crisis under a new federal Department of Homeland Security directive. The drill was to resemble the Columbine High School tragedy, with four active shooters on each floor with hostages taken and injured persons still inside the school.

Students and other residents volunteered to act as casualties and extras in the drill. Some students were given fake wounds to wear by medical staff from MacNeal Hospital in Berwyn, who then practiced their triage skills with waiting ambulances.

The training exercise was a joint effort between the Cicero Police and Fire departments, Mutual Aid Box Alarm System Division 11, Cicero Elementary School District 99, the Cook County sheriff's police department, Berwyn's MacNeal Hospital and the Berwyn Police Department.

Neighbors and the media were held at bay as riot-geared police teams swept through the four-story building, selected for the drill because of its one-block-long size. The school is the largest junior high in Illinois.

"We're still dealing with the hostage situation on the fourth floor," said another officer walking out of the school. "There's an armed subject barricaded in a classroom with hostages. That's for the county."

Outside, a beige military tent was propped as an improvised triage unit in the corner of the school's parking lot. The mock victims -- some slashed across the cheek in fake red dye, others with torn shirts at the abdomen to indicate a gunshot blast -- were set down on the grass.

"We're a level two trauma center," said Jennifer Flanagan, trauma coordinator for MacNeal. "We want to practice these drills out in the community. We need to know what we're doing internally is going to help the community when there's a disaster."

I know as part of FEMA drills they will be practicing "drive-thru" shot clinics. They are supposed to be using Walmart parking lots mall lots etc. I wonder if this could be a set up for the big FEMA exercise coming soon? Too soon for the actual shot clinics but it might be a dry run.

Over the last several years, the Collin County Commissioners Court has awarded "no bid" contracts in excess of $1.2 million to ADB Consulting of Santa Fe, New Mexico. ADB is owned and operated by Dr. Bob Johnson and his wife, Anita Miller. Dr. Johnson is the son of US Congressman Sam Johnson of Plano.

These "no bid" contracts awarded federal and state grant funds to ADB for construction and operation of the North Texas Fusion System.

The Fusion Center is a high-tech data center that collects, analyzes, and distributes data on possible terror and environmental threats. It is operated by Collin County, but mostly funded with federal homeland security grants.

Stung by criticism of insider dealings for not requiring ADB to submit competitive bids, the commissioners court in February put the contract for running the Fusion Center out for bid. WFAA's Brett Shipp reported that the county's Homeland Security Director, Kelley Stone, acknowledged that Dr. Bob Johnson helped to write the bid specifications, even though ADB planned to submit a proposal.

The closing date, when all bids were required to be submitted, was March 19, 2009.

Yet almost four months later, no contract has been awarded.

Two months ago, I asked the county's purchasing agent, Frank Ybarbo, what was holding up the award. "Were any qualified bids received?," I asked. Ybarbo told me that the county had indeed received five qualified bids. He confirmed that one of the five bidders was ADB Consulting.

Ybarbo told me that when bids are opened, the affected county department managers analyze them, and may take an option to further negotiate with one or more of the bidders. Nevertheless, Ybarbo thought that the bid would be ready for the Commissioners Court to make an award in late June.

The affected department head is Kelley Stone, who was the one who brought ADB and Dr. Bob into the Fusion Center. Stone has admitted that before now, no other vendor had been given an opportunity to quote work for the Fusion Center.

In fact, back in 2007 when another vendor, Sypherlink, attempted to bid on subcontracted work for ADB at the Fusion Center, its bid was thrown out in favor of another insider deal -- this time for a company headed by Dr. Johnson's brother-in-law. Sypherlink wrote a letter to Kelley Stone and Frank Ybarbo, but the county took no action to enforce or report the alleged federal nepotism violations.

Federal grant money however, keeps rolling in. Recently, Congressman Ralph Hall submitted an earmark that would evade normal grant writing procedures and give the Fusion Center another $500,000 in federal funds for "Completion of the North Central Texas Fusion System."

After having a history of insider dealings with one politically well connected vendor (ADB) who actually helped write the bid specifications, the county has spent over 100 days "negotiating" rather than making an award.

There's a rotten smell beginning to come from this whole deal
