My daughter just started 6th grade (middle school). All sorts of forms were sent home for parents to fill out, including medical info.

I made a note to send in with the form that said that my child, name, is not to receive any type of shot or vaccination without the permission of her father, name.

This should, atleast, inform them that I am opposed to any type of vaccination. Hopefully they will be more inclined to contact me regarding this. Thoughts?
